• You must complete a questionnaire for each of the categories you have been shortlisted in
• Questionnaires must be completed online via this portal
• The submission deadline is Friday 7 July, 23.59 BST.

• Winners will be invited, along with their colleagues, to attend the awards ceremony on 23 November at Magazine London
